Do I need a doctor's prescription or referral?
   
Yes, a doctor's prescription is required for physical therapy.  Please
bring your script to your first appointment. Without your prescription, we
can perform an initial evaluation, but will not be able to begin treatment.

What should I expect on my first day of physical therapy?
   
You should plan to arrive about 15 minutes before your scheduled    
appointment time.  When you arrive, you will be asked to fill out              
several forms about your personal information, health insurance,             
past medical history, and goals for therapy.  Once your paperwork is         
completed, you will be evaluated by one of our physical therapists.            
You should bring or wear loose, comfortable clothing.  You can                  
expect your first appointment to last approximately 1.5 hours, but               
this can vary from person to person.

How long will I be in physical therapy?
   
The duration of a client's treatment can vary greatly.  The length of
time spent in therapy can be influenced by diagnosis, severity, healing,
other illnesses, and many other factors.  Your therapist can discuss your
individual situation with you, and help keep you updated regarding your
progress during your sessions.

What if I do not have health insurance?
   
SCORE offers self-payment options, and is willing to work with you to
set up a payment plan that you can afford.  Please contact our office for
more information.

What if SCORE does not participate with my insurance company?
   
If we do not participate with your insurance company, you can check to
see if you have any out of network benefits through your                   
insurance.  If not, please contact our office for additional options.

Are there any discounts available for the Wellness Center?
   
Yes, we do offer discounts for the Wellness Center.  We offer           
post-physical therapy and group discounts.  Please

I have never really worked out before.  Can someone help me to
get started?
   
At SCORE, a personal trainer will create an individualized program for
new Wellness Center members.  They will also give you a tour of the
Wellness Center and instruct you on the proper use of the        
equipment. Our staff of physical therapists and personal trainers are         
always available to answer your questions at any time.
